Telehealth And Visting Specialist Center :: Provider Bios

Linda Lawrence Cade CDE, FNP, MSN

Linda Cade is our diabetes education specialist and Chronic Care Coordinator for Open Door Community Health Centers. She serves as senior leader for their Federal Health Disparities Collaborative efforts in diabetes and depression. She is also a Family Nurse Practitioner working at two of the network clinics. Linda received her BSN from UCSF in 1978 and her FNP and MSN from California State University, Sonoma in 2000.

Linda has worked with diabetes patients throughout her career, and has been actively involved in the Humboldt Diabetes Project since its inception. She has taught the Insulin BASICS curriculum for several years locally, and through the TVSC, she provides diabetes education and diabetes management consults for single patients or groups, both in person and via telemedicine. Her areas of expertise include the implementation of network and community wide disease registries and health education programs as well as the use of the Chronic Care Model as a framework for disease management.
