Family practice services for all of your primary medical needs, including diagnosis, treatment and referral for more advanced care as necessary. Clinic provides diagnosis and treatment for acute illnesses (such as flu, colds, aches and pains, fatigue) and chronic conditions (including diabetes, high blood pressure, asthma, emphysema). Routine health screenings, immunizations and boosters, and lab tests are provided as appropriate. We work to help you with disease prevention, health maintenance activities and ways you can take care of yourself. Well-child examinations and annual adult check-ups offered to all patients.

Health Access Assistance Program (HAAP): HAAP staff available to all DNCHC patients for free assistance services including application assistance for public insurance programs and information on available community resources. HAAP Office at the Wellness Center, Suite 200. Monday-Friday 8:30am-6:30pm and Saturday 9:30am-1:30pm. Walk-In and appointments accepted. (707) 465-1988

Additional services at the Del Norte Community Health Center include: Pediatrics, Behavioral Health Counseling, Diabetes Education, Nutritional Counseling, Teen Clinic (see, HIV/AIDS testing and treatment, Hepatitis C treatment, Nutritional Counseling, and Women’s Health. Patients established at the Del Norte Community Health Center can be referred to other Open Door clinics, including the Telehealth & Visiting Specialist Center, for additional and specialty services. Referral to and coordination with other specialists are provided as needed, with follow-up care in the clinic. The clinic also provides follow-up care after hospital stays. The clinic has 20 private examination rooms, with five physicians, two mid-level practitioners, and a clinical psychologist on staff.

Young children should be seen often for immunizations and regular well-child examinations (our pediatrician can explain the best schedule of visits). Older children and adults should be seen for routine check-ups every year. Public or private coverage is typically available for children. Coverage for adults depends on a number of factors; we will assist you in determining eligibility for Medi-Cal, Medicare, CMSP, Family PACT and other programs. Those without insurance are eligible for discounts based on income and ability to pay. We accept most private insurances.

Amber Vos, FNP, joined Open Door Community Health Centers in the Summer of 2012.

She is a graduate of Western University of Health Science where she earned her Master’s Science in Nursing Family Nurse Practitioner in August 2011.

Christian Holland, DO
Christian Holland, DODoctor of Osteopathic Medicine
Dr Christian Holland joined the Open Door Clinics in 2009. He obtained his D.O. degree in 2005 from Western University of Health Sciences in Pomona, CA. and completed his Internal Medicine Residency at Loma Linda University Medical Center in 2008. He went on to complete a Chief Residency year at Riverside County Regional Medical Center in 2009.

Dr. Holland feels privileged to be part of the National Health Services Corps, which is a scholarship designed for those committed to providing primary health care to underserved communities. His academic interests include clinic-based and hospital-based internal medicine. His personal interests include running, hiking and enjoying the majestic beauty of the Redwoods and northern coast line.

Dr. Pearson, a recent addition to the Open Door team in Del Norte County, is a Northern California native who graduated from UC San Diego School of Medicine in 1996, with completion of his Residency in Family Practice from Sutter Merced Medical Center in 1999.

Dustin Jacot, PA, joined Open Door Community Health Centers in the Summer of 2012. He is a graduate of University of California, Davis, where he earned his Physician Assistant Degree in June 2012.
Katrina Groves, MD
Katrina Groves, MDDoctor of Medicine
Dr Katrina Groves, Originally from the southern California area, graduated from the medical school of Pennsylvania in 1989 and returned to California for her 3 years of residency in Family practice. In August of 1992, she relocated to Del Norte County under the National Health Service corps program. She and her husband (Dr. Rehwaldt) planned to serve their time and move on, but soon found they loved the area. With its unspoiled beauty of oceans and redwoods, and the absence of smog or traffic and other “big city” problems, they decided to stay and raise a family.
Dr. Groves has been dedicated to the under served patients of our area and to the Open door clinic for over 15 years. Her special interests in medicine include women’s health and she is a member of the American society for Colposcopy and Cervical Pathology. She is also certified with the American Academy of Pain Management. In her leisure time she enjoys making and firing her own pottery, learning to play the viola, hiking in the redwoods, and swimming the rivers and ocean with her two sons. She feels privileged to care for and be a part of her patients’ lives.
Warren Rehwaldt, MD

Dr. Warren Rehwaldt, a Midwestern native, completed his medical school training from the University of Minnesota in 1989. He met his future wife, Dr. Katrina Groves, while completing his family practice residency in San Bernardino, California. Together they moved to de Norte County in 1992 where he served for 5 years as the first full time physician for United Indian Health services in Del Norte County. He went into private practice for 4 years, eventually joining the Open door team in 1999. from 2001-2005, he served both as the county Public Health Officer and staff physician at the Del Norte community health center. He worked tirelessly in the community to maintain quality care during the severe provider shortage in Del Norte County. Dr. Rehwaldt became the Site Medical Director of DNCHC in 2005 and has been heavily involved in recruitment and development plans for the beautiful new Open Door facility in Del Norte County, the Wellness Center. He has served as vice chief of staff twice for Sutter Hospital and remains active there. His professional interests include Adult and Geriatric Medicine, as well as Pediatrics, and his main role in the last 2 years has been to provide hospital care for all ages.
His personal interests include music (he plays a variety of stringed instruments and is a member of the local orchestra), reading, cooking, salmon and steelhead fishing, and just about anything that can be done outside – but mostly spending as much time as possible with his children!
