Beginning January 2016, Open Door needs to verify income documents for patients eligible for Sliding Fee Scale (SFS) discount, regardless of insurance status. We screen you for programs you may be eligible for and give you information to help you apply.
If I have insurance why do I need to verify my income?
Eligibility for SFS is based on income level, not on insurance status. Anyone with income at or below 200% Federal Poverty Level (FPL)* is eligible for SFS, even if you have insurance. If you are qualified, SFS may include some services your insurance may not pay for, or apply if you have a large deductible or share of cost. If you choose to decline SFS for today’s visit, any charges not covered by your insurance for today’s visit will be charged full cost to you even if you apply for SFS at a later date.
If my income is over 200% FPL why am I asked for my income information?
When you provide your income information you assist Open Door in collecting data we use for grants to support our services. If your FPL is over 200% you are not eligible for SFS and do not need to provide income documents even when you provide your income information. We thank you for your support of our programs.

Determinaremos a cuales programas usted es elegible y le daremos informaci6n para ayudarle a aplicar.
¿Si yo tengo seguro medico porque necesito verificar mi ingreso?
La elegibilidad para SFS esta basada en el nivel de ingresos, no en el estado de su seguro. Cualquier persona con ingresos de o por debajo de 200% del lndice Federal de Pobreza (FLP por sus siglas en ingles)* es elegible para SFS, inclusive si usted tiene seguro medico. Si esta calificado, SFS puede incluir algunos servicios que su seguro medico no incluye, o aplica si usted tiene un deducible grande o cuota de pago compartida. Si elige rechazar SFS para la visita de hoy, cualquier cargo no cubierto por su compafHa de seguro medico para la visita de hoy le sera cargado el costo del valor completo inclusive si aplica para SFS en el futuro.
¿Si mi ingreso esta por encima de 200% FPL porque me preguntan por mi informaci6n de ingresos?
Cuando usted provee su informaci6n de ingresos usted ayuda a Open Door en la recolecci6n de datos que usamos para los fondos que ayudan nuestros servicios. Si su FLP esta por encima de 200% no es elegible para SFS y no es necesario proveer documentos de ingresos incluso cuando usted nos dio informaci6n sobre sus ingresos.
