Our Mission Statement
Open Door Community Health Centers promotes social justice and human dignity through exceptional patient-centered care that improves the health and well-being of our patients, community, and staff.
Open Door Community Health Centers was founded with a vision to provide excellent health care and health education to a part of California, at a time when the region suffered from a shortage of access.
Starting as a single clinic in 1971, Open Door now has 12 community health centers across Humboldt and Del Norte counties – a service area larger than the state of Connecticut – serving more than 55,000 patients a year and employing almost 600 members of the community.

Patient-Centered Care
Patient-Centered Medical Home
A patient-centered medical home (PCMH) is a model of patient care that uses evidence-based practices and follows a team-based approach that emphasizes care coordination and communication across specialties to achieve an excellent patient care experience and help lower medical costs.
The United States Preventative Services Task Force (USPSTF) is one of the many organizations Open Door refers to for evidence-based guidelines.

The members of your clinical Care Team meet daily in what we call “huddles”
to prepare for patient visits.
Why do Care Teams huddle?
- To support communication between Care Team staff.
- To keep a close eye on patient coordination of care.
- To make sure your visit goes as smoothly as possible.
What does your Care Team talk about in a huddle?
- They review the reason(s) for your visit.
- Decide what types of care and/or services will be completed.
- Find opportunities to update preventative care needs, such as cancer screenings and vaccinations.
How does your Care Team prepare for the huddle?
- You Care Team completes “pre-visit planning” to review your chart and prepare for a phone call to you to confirm your appointment.
- Your Care Team will call you 1-3 days before the appointment to confirm AND go over the plan for the visit so both you and your Care Team are on the same page about the upcoming visit.
Open Door Community Health Centers is an independent, non-profit community health center, led by an independent and consumer-majority board of directors, licensed by the State of California.
Open Door is a Federally Qualified Community Health Center (FQHC) Program grantee under 42 U.S.C. 254b and receives funding from the U.S. Department of Health & Human Services (HHS).
Open Door has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims (including medical malpractice claims) for itself and its covered clinicians/practitioners and support staff.
