Vaccines at Open Door
Open Door patients 6 months and older can schedule a vaccine appointment via MyChart.
To make a MyChart account, go to the Open Door MyChart signup page.
Log into your MyChart account and click “Schedule An Appointment.”
Patients are required to bring their immunization card to their vaccine appointment.
